The Patient Care Coordinator oversees the implementation of standard work within the clinical setting of all integrated primary care clinics. The Patient Care Coordinator is in charge of front office standard work and works to help clinics to achieve clinical excellence in patient care.
This is a telecommuting position, full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to be available during normal business and clinic hours of 8:00am - 5:30pm MST. It may be necessary, given the business need, to work occasional overtime. This position requires 80% of clinic travel and the other 20% will be spent working from home.

Primary Responsibilities:


  • Review processes and support the practice staff with standardized work
  • Work cooperatively with all care team members to support the organization's vision and mission, deliver excellent customer service, and adhere to Lean processes
  • Assist coworkers as needed to assure smooth office operation and delivery of excellent service through teamwork
  • Provide ongoing support for the practices for resource integration efforts
  • Using established tools and processes, act as key contact between new clinics and operations team to implement new front office setups; support different implementation scenarios for new and existing clients
  • Proactively seek and implement process improvements including analyzing data trends to identify opportunities through internal and external system/process enhancements
  • Create phone scripts for incoming and outgoing phone calls and in-person questions related to the change in name, acquisition or to address FAQ that are in line with standard practices
  • Support staff in updating provider templates/procedures to align with standard practices and ensure correct scheduling time is addressed in 20/40 minute time slots
  • Provide tools and resources that pertain to patient follow-up to identify and close gaps in care and align with standard practices
  • Provide resources and documents to care staff to connect patients with outside resources in the community, social services and facilitate actions to promote self-care
  • Develop standardized practices for insurance eligibility/benefits and guide in seeking additional information, keeping patient credentialing up to date office to office, providing resources to fill in any gaps
  • Update and standardize patient demographic forms, patient forms and consent forms to be used to upload into computer systems
  • Generate standard workflows for inputting patient data through relevant computer systems/applications such as EOD, check and deposit handling, patient follow up and standardized work front desk
  • Create standard workflow for generating patient/staff data and aggregate data reports
  • Monitor and guide front desk staff and care coordinators on standard practices, upcoming changes, and standard workflows
  • Outlining current partners, who they are, where to find information pertaining to these resources, and what contact information to provide to patients
  • Provide adequate contact information, outline needs to promote open communication and provide information to patients that promotes increased rapport for physicians and staff
  • Outline and ensure a basic understanding of co-pay, payments, plan guidelines, benefits and medical insurance, Medicaid/medical, commercial benefit plans, and federal/local government regulations
  • Perform other duties as assigned

      • High School Diploma/GED (or higher) -
      • 1+ year of medical front office experience
      • Intermediate level of computer proficiency (multi-task through multiple applications including Microsoft Outlook, Excel, and Word)
      • Ability to travel up to 80% of time to clinics and remainder working from home

        Preferred Qualifications:

        • 1+ year of related healthcare experience to include scheduling experience -
        • Knowledge of medical terminology
        • EMR experience
        • Office Administrative / Receptionist / Clerical experience in a medical environment

          Soft Skills:

          • Excellent communication skills and phone etiquette -
          • Strong attention to detail
          • Ability to multi-task -
          • Ability to manage time effectively and work independently.
