StartDate: ASAP
One of Southern Californias leading healthcare systems is seeking
family medicine nurse practitioners to work at West Covina or
Commerce, CA clinic locations as a transition of care and primary
care hybrid provider. The successful candidate will work alongside
an interdisciplinary team. Candidates must be certified.
The typical schedule is 4 ten-hour shifts with the varying
transition of care and primary care duties splitting the nurse
practitioners day.

Improved patient experience: Patients can receive care
seamlesslythrough the continuum, minimizing caredelays and adverse
events
Better health outcomes: Decrease adverse events related to
medications and access to care
Lower cost of care: Each readmission costs $6000 in institutional
fees for AHN. Additional savings in professional fees, pharmacy, ED
utilization, etc.
Improved staff experience: Ensures providers know about care
outside their four walls and thus havegreater control of their
panel
