Job Description:

Job DescriptionResponsibilities The incumbent works with Advance Practice Provider Dementia Care Specialists (DCS) in support of the UCLA Alzheimer's and Dementia Care Program (ADCP). Coordinates and manages data from various ADCP projects. Contacts patients and patient's families to follow-up on DCS' orders. Schedules telephone, video or in-person appointments with the DCS, as appropriate. Documents under the minimum supervision of the DCS in the electronic medical record. Oversees the management of patients in lower acuity levels of need by performing needs assessment and applying program interventions per protocol. Monitors and interprets patient's and families' needs by identifying families in crisis and follows protocols to ensure that their needs are met. Analyzes and scans patient visit's paperwork and tools. Communicates professionally and compassionately via telephone and via email. Must be proficient in English. Makes referrals to appropriate community-based services and applies non-pharmacologic management of dementia-related behaviors and caregiver education/support. Contacts patients/caregivers to determine whether they have needs that require DCS services. Meets weekly with the supervising DCS to summarize interventions instituted for these patients. Participates in data entry, analysis/ interpretation and reporting. Works collaboratively with the DCS to ensure that patient acuity assignments are accurate and up-to-date. Qualifications --- Demonstrates proficiency with computer skills (MS Office Suite), document scanner and other software --- Ability to learn electronic medical record system to document patient notes and produce reports --- Fluent in communicating verbally in English --- Able to apply care management protocols to patients/caregivers, as appropriate. --- Ability to analyze and relay potential concerns to Dementia Care Manager, as appropriate --- Ability to meet deadlines and to follow through on multiple tasks with careful attention to detail and with minimum supervision. --- Demonstrated writing skills to draft routine business correspondence and meeting minutes and to edit into final form. --- Demonstrated editing skills to identify errors in grammar, syntax, punctuation and spelling. --- Interpersonal skills to effectively develop and maintain cooperative working relationships with faculty, administrators, students and with elderly patients. --- Attention to detail, in order to accurately enter and manage database for ADCP UCLA is an Equal Opportunity/Affirmative Action employer.
