RN Outpatient Care Manager for a travel nursing job in Los Angeles,
California. Job Description & Requirements - Specialty: Care
Manager - Discipline: RN - Duration: 8 weeks - 40 hours per week -
Shift: 8 hours, days - Employment Type: Travel Unit: Office of
Diversion and Re-Entry (ODR) Candidates must be fully committed to
completing the 8-week assignment - No time off approved during 8
week contract. Responsibilities: • Review and triage medical
incident reports • Coordinate support for patients and community
care sites • Communicate with facilities via phone/email • Assist
with referral review, discharge coordination, and placement support
Requirements: • Active California RN license on hand • 1 year RN
experience • Strong clinical judgment & communication • Case
management or community care experience preferred
