Responsibilities - Deliver skilled nursing care to home health
patients with acute care needs - Complete accurate and timely OASIS
documentation - Manage a high-volume caseload of approximately 5–6
patient visits per day - Collaborate with interdisciplinary care
teams to ensure quality outcomes - Travel within assigned territory
to provide in-home patient care Required Qualifications - Active RN
License (Pending licenses not accepted) - Minimum 2 years of Home
Health RN experience - Recent OASIS experience REQUIRED -
High-volume caseload experience REQUIRED - Previous Home Health
Travel Experience REQUIRED - (General travel experience not
accepted) - Ability to pass an OASIS competency exam Preferred
Qualifications - Experience with MatrixCare EMR - Strong time
management and organizational skills - Familiarity with LA County
