MDS Coordinator to join our team!Imperial Care Center is a
dedicated Alzheimers Dementia Care Facility, offering a high
quality of life provided by specially trained, Loving Care Givers
for people with memory loss.The MDS Coordinator completes MDS for
all residents according to OBRA and PPS guidelines. Assists in
providing a clean, safe, happy and healthy environment for
residents by performing the duties as described below. The goal is
to promote the patients well-being by providing high quality
nursing care.SCHEDULE: MONDAY THROUGH FRIDAY (AM)MUST HAVE SNF
EXPERIENCEMDS Coordinator RequirementsValid and current California
RN or LVN License1-2 years MDS experience or MDS completed
courseBLS or CPR (required)Fully vaccinated against COVID-19
including Booster shots1-2 years experience in MDS coordinator role
or related experience requiredExceptional mathematical skills and
ability to apply them to tasks such as analysis of variance and
sampling theoryDeep medical terminology knowledgeExperience in a
clinical and healthcare setting preferredExcellent leadership
skillsDemonstrable knowledge of state and federal regulationsSuperb
decision-making skillsPrevious experience in a SNF or similar job
setting would be preferredExcellent knowledge of nursing care
methods and procedures.Excellent knowledge of emergency
care.In-depth knowledge of health and safety guidelines and
procedures (sanitation, decontamination etc.) and willingness to
follow them at all timesA team player with excellent communication
and interpersonal skillsPrimary MDS Coordinator
ResponsibilitiesParticipate in the admission process of prospective
residents in terms of their nursing needs and appropriate
placementDetermine potential Resource Utilization Groups (RUGs) and
expense associated with a potential admissionReview all
applications for admission, followed by on-site or phone assessment
and communicate results of assessment to the Admissions
CoordinatorComplete and assure the accuracy of the MDS process for
all residentsMaintain current working knowledge of Medicare
criteria, serving as a resource for nursing staff and communicate
changes in regulationsMonitor Case Mix Index (CMI) scores, looking
for potential risks and/or changes that may affect Medicaid
reimbursementFacilitate problem-solving for complicated
admissionsMonitor Medicare assessment schedules and nursing
documentation to ensure accuracy and timely submissionComplies with
the facilitys attendance policy requiring regular, reliable, and
punctual attendanceOther ResponsibilitiesAdjust and administer
patients medication and provide treatments according to physicians
ordersEnsures timely response to call lightsPain
management/interventionAssess behavior and provide interventions as
indicatedWound treatmentsTherapy treatmentsIncidents; falls,
etc.Patient complaints / concernsIdentifies/assesses emergency
medical situations and provides appropriate interventions as
indicatedDocumentation and follow-up with timely notification of
physician and familyCare planning as neededPerforms other duties or
functions as assigned by the DON and/or AdministratorPAY
SCALE:Depending on a number of factors, including, but not limited
to location, regularly scheduled work shift, knowledge, skills,
