The Clinical Social Work Supervisor position will be assigned to the DHS Patient Centered Medical Home (PCMH) known as the Star Clinic. This clinic is a specialized PCMH for patients who are homeless or formerly homeless and who have complex health and behavioral health conditions. Under the direction of the Social Worker Chief, the Clinical social work supervisor will provide oversight of behavioral health services and case management services to patients who are eligible for enhanced care management (ECM) at the Star Clinic. Clinical Social work supervisor will oversee the behavioral health unit and in partnership with the SW chief, plan, direct, assign, and evaluate the work of professional social work and support staff including Community Health Workers, Medical Caseworkers and Substance Use Counselors. Confers with the STAR Clinic team leadership to plan new programs and coordinate the Behavioral Health Service Delivery to STAR clinic patients and clients.

The Star Clinic is under the general supervision of the Housing for Health Medical Director, Senior Physician, who receives administrative supervision by the Director of Housing for Health and clinical supervision from the Deputy Director of Community Programs.

ESSENTIAL FUNCTIONS

  • Provide supervision and oversight for Star Clinic ECM behavioral health team
  • Direct Report/Supervision will include Clinical and Administrative oversight for following staff on Star Clinic ECM behavioral health team:
    • Sr. Clinical SW (2)
    • Medical Case Workers (2)
    • Substance Use Disorder Counselor (1)
    • Community Health Workers (6)
    • Supervision to include job assignment, performance management/evaluation and training of staff. Will provide oversight to ensure there is optimal care coordination and continuity of care for the Star Clinic ECM patients, including:
      • Coordination, facilitation & participation of ECM care coordination meetings o Work with staff to promote care integration with ECM team, Star Clinic staff, inpatient care teams, intensive case management services (ICMS), and Housing for Health's home nursing program.

        o Provide consultation to intensive case management services (ICMS) and Star Clinic staff

        o Assist with the development of workflows and team policy and procedures.
        • Provide support to staff with debriefings, incident reports, etc
        • Plans and conducts unit orientation and continuing staff development program and participates in the orientation programs of other disciplines
        • Coordinates the activities of clinical social work services in the unit with other disciplines through interdisciplinary staff meetings, resolution of interdisciplinary procedural and personnel problems, and coordinated implementation of new services or programs
        • Ensures appropriate training and competent service delivery related to Substance Use intervention and treatment, Mental Health Service delivery models, and addressing Social Determinants of Health (food insecurity, housing, etc.)
        • Reviews and revises departmental policies interprets Federal, State, County and departmental policies and regulations for social work to department staff
        • Work with staff to promote care integration with outreach to workforce and other treatment teams
        • Provides consultation to the multidisciplinary ECM teams to support management of chronic medical and behavioral condition and prevent acute care utilization through transitions of care interventions
        • Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services ADDITIONAL ADMINISTRATIVE FUNCTIONS
          • Assist with the identification of training needs and the development HFH BH team Training
          • Reviews and revises departmental policies interprets Federal, State, County and departmental policies and regulations for social work to department staff
          • Support SW chief in activities related to the build out and development of Behavioral Health program and services for HFH
          • Support to SW Chief in macro activities as needed for HFH programs, including orientation and work force development/training, workflow and policy development
          • Support the implementation MSW intern and field placement program
          • Provide input regarding program, policies and procedures through individual conferences, staff meetings and budget requests
          • Participates in performance/quality improvement (PI/QI) activities and contributes to policy development as needed
          • Shares knowledge and effective practices with other Care Team members. DESIRABLE QUALIFICATIONS
            • Clinical social work in Homeless health care experience
            • Experience/expertise in assessing and managing clients with dysfunctional SUD Experience/expertise in assessing and managing clients with severe persistent mental illness
            • Experience/expertise in trauma-informed care and harm reduction principles
            • Demonstrated teaching and communication skills with clinical and non-clinical staff
            • Experience providing clinical and administrative supervision to other behavioral health staff preferred
            • Familiarity with DMH, DPH and community mental health and substance use programs and resources EDUCATION/EXPERIENCE

              A Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education followed by four years of professional social work experience, two of which must have been in a medical setting functioning in an independent capacity providing highly specialized social work services.

              Certificates/Licenses/Clearances
              • An active and valid license as a Licensed Clinical Social Worker issued by the California Board of Behavioral Sciences
              • The required license MUST be current and unrestricted; conditional, provisional, probationary, or a restricted license will not be accepted
              • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions
              • Successful clearing through the Live Scan process with the County of Los Angeles
