About this Role
This is an opportunity for a mission-driven, team-oriented clinical leader who is passionate about delivering care to frail and disabled older adults in the PACE model. In this role, you will be responsible for overseeing and leading the effective management and delivery of direct nursing care to PACE participants in a variety of settings, including but not limited to the center, participant's home, virtually, SNF and beyond. You will be a key member of the Interdisciplinary Team (IDT) and work collaboratively with the Center Director to carry out administrative duties as needed. This leader also serves as acting Center Director when the Center Director is absent or unavailable.
Expectation for this role

  • Provides program level guidance and recommendations relevant to the provision of clinical and nursing services
  • Monitors and evaluates therapeutic interventions
  • Delegates responsibilities to other IDT members and oversees quality of their care as appropriate
  • Accepts "on-call" duty on a rotating basis and assists the Site Director with scheduling the rotation.
  • In addition, with initial opening of the PACE site, the Director of Nursing may also serve as a clinical RN with responsibilities including:
  • Participates as a member of the IDT, and assisting in completing sections of assessment and plans of care along with service determination requests
  • Assist in managing a panel of participants with a medical provider a point and being the point person for the IDT to ensuring progress on a participant's plan of care
  • Reviews and implements physician's orders
  • Conducts nursing assessments, inclusive of physical, psychosocial, and behavioral status
  • Educates participants, caregivers, and staff
  • Oversees and delivers care to wherever the participant is located (the home, center, virtual, ALF, SNF)
  • Monitors and evaluates therapeutic interventions
  • Delegates responsibilities to other IDT members as appropriate Attributes of an Ideal Candidate
    • Passion and mission orientation for serving high-risk seniors and frail older adults
    • Minimum of 5 years' experience caring for adults, chronically disabled and frail or elderly populations in a healthcare setting. Ambulatory care experience is helpful but not essential
    • Prior PACE experience is preferred although not necessary
    • FunctionAble to function independently in the day-to-day management of the clinical needs of participants while being able to work collaboratively with the interdisciplinary team in meeting the goals of the participants.
    • Active listening and conflict management skills
    • Can quickly adapt to multiple clinical settings
    • Training in wound care, IVs, blood draws
    • Active, unrestricted RN license in California
    • Graduate of an accredited School of nursing, BSN strongly preferred
    • CPR and Alzheimer's certification
    • Languages Spoken: English and Spanish preferred Location
      • Los Angeles, CA
      • Ability to be 20-100% physically present within our clinics
      • This position is hybrid (in office and work from home)
      • Up to 80% travel may be required
