Job Number: 5369 Job Description We are seeking master's level
social worker Care Coordinator roles in S California. These are
temporary assignments that are expected to last 3+ months at a
large healthcare system in with locations across S. Ca. The
position will be coordinating the care for defined patient
populations in order to plan and implement a care plan. This is a
telephonic, hospital-based position that will be M-F, 8:30 am-5 pm
or 9:00 am-5:30 pm. No evenings or weekends are required.
Pays $35 an hour.
Qualified candidates must have a master's degree in social work
(MSW) with an ACSW registration a plus!. Must have social work
experience in the medical field. Excellent communication skills are
a must. The hospital setting requires a Covid vaccine and
booster.
