Job Description:
POSITION SUMMARY The Physical Therapist provides therapeutic
services to patients with impaired muscle, joint, and functional
impairment in accordance with physician's orders. SPECIFIC SKILLS
NEEDED Cognizant of home health standards and practices, including,
but not limited to assessments specific to discipline and
documentation requirements. Basic computer knowledge and abilities.
Electronic medical record input experience preferred.
EDUCATION/EXPERIENCE/TRAINING Graduate of an approved program in
physical therapy and licensed as such by the P.T. Examining
Committee of the CA Board of Medical Quality Assurance. Current
California physical therapy license. Current CPR card. Current
California driver's license and valid auto insurance. Minimum 2
years experience as a physical therapist. DUTIES AND
RESPONSIBILITIES Safeguards and preserves the confidentiality of
patient's protected health information in accordance with State and
Federal (HIPAA) regulatory requirements, hospital and departmental
policies. Ensures a safe patient environment and adherence to
safety practices per policy. With consideration to age, employee
utilizes the approved process to resolve biophysical,
psychological, educational and environmental needs of
patient/significant other when administering care. Essential to the

to management positions.) Assists the physician in evaluating
patient's level of function and reevaluating as appropriate by
applying diagnostic and prognostic procedures. Provide treatment by
means of graded and therapeutic exercises. Promote functional
ability toward increased independence in patient's home
environment. Assist in developing and updating the Plan of Care.
Provides initial PT evaluation or SOC OASIS collection as indicated
by referral type. Observing, recording and reporting information on
the patient's condition to the attending physician and in the
patient's clinical record. Advising, consulting and, when
appropriate, instructing family and other department personnel in
patient's therapy program. Teaching and supervising other health
personnel when appropriate. Documents according to department
policy, procedure and standards. Provides in-services on PT
services as requested by Clinical Director. Performs any and all
duties as assigned by supervisor. Employment Type: Full Time Years
