We provide Primary Medical Care, Mental Health Services, Social Support, Re-entry, and Substance Use Disorder Treatment. We have 4 separate clinic locations in in the community that provide an integrated model.--Each clinic has its own unique history of serving a wide patient base from African American, API, Co-Occurring Disorders, HIV/AIDS, Homeless, Incarcerated, LGBTQ, MSM, Parolees, Probationers, Women and Children, and Youth and Young Adults.

We are currently looking for 3 clinical positions including Internal and Family Medicine staff, Medical Director, and Deputy Medical Director positions.

Job Details:

• Provide--preventive-oriented--ambulatory--primary--care--as--part--of--an--integrated--health--care--team--in--a patient-centered health home--delivery--model
• Provide--primary--care--to--those--with--chronic--disease,--addiction, and--mental--illness
• We have medical students, residents, and fellows rotating through, and the medical director may have an opportunity to get a adjunct professor title through UCSF
• Outpatient-only with telephone call
• Between 1:4 to 1:5 call with minimal telephone only
• Patient population is 18 years of age an higher
• 18 patients per day (total patient panel of 1,100) --
• EHR --- E-clinical works
• Practice is integrated and has behavioral health with 1 Psychiatrist and 2 LCSWs (1 part-time)--
• 80% of the patients for the clinic come from substance abuse, mental health, criminal justice system, homeless, or near homeless
• Assist--Medical--Director--in--supporting--mid-level--practitioner--and--review--mid-level--practitioner--and/or--peer-physician--charts
• May--act--as--clinical--lead--in--the--absence--of--the--Medical--DirectorBenefits:

    • Understand social determinants of health, impact on health, and referral to support addressing patient needs regarding determinants
    • Integrated team setting
    • ?Comfortable with and has experience in transgender health
    • Familiar with treating issues like addiction
    • Needs of individuals experiencing homelessness, HIV, and Hepatitis C
    • The ideal candidate is also comfortable treating chronic disease and mild to moderate mental illness
    • In addition to advocacy and care for an underserved population, you will gain increased knowledge of addiction treatment across a continuum of care
    • You will also learn about integrated care beyond the clinic to include substance use disorder, mental illness treatment, and social determinates of healthMedical Director Position--
      • Will have oversight of the clinical management of the practice and direct management of 1 Physician, 2 Nurse Practitioners, and a PA
      • 40% to 50% administrative duties,--including 4 to 5 half days, seeing an average of 10 patientsDeputy Healthcare Officer
