The VIP Community Mental Health Center (VIP CMHC) is an exempt 501(c)3 nonprofit organization that works under the umbrella of the greater Violence Intervention Program (VIP). VIP CMHC offers mental health services for children, 0 to 25 years old, and their non-offending family members living in LA County. VIP CMHC provides treatment using a multidisciplinary treatment team, and it also integrates medical and mental health assessments to provide comprehensive treatment to the child. Its clinical staff provides services on- site and in schools as well as at the VIP's Forensic Medical Clinic as children are triaged after trauma.Violence Intervention Program is a non-profit organization dedicated to serving the community in the heart of Los Angeles County. Every employee knows that they can make a difference and change a life.POSITION SUMMARY Located at the LAC+USC Medical Center HUB Clinic, the Client Care Coordinator/Case Manager is responsible for providing support services to patients. This position will maintain and complete all administrative record keeping associated with the ACT clinic, specifically for the Alexis Project. The Alexis Project is a partnership between The Alexis Arquette Family Foundation and Violence Intervention Program (VIP) at the LAC+USC Medical Center, and is an extension of VIP's hallmark medical, mental health and support programs serving the LGBTQ+ population. Transgender and gender expansive individuals can find safety at the Alexis Project with specialized medical and mental health staff who understand their needs. We're working to normalize health and mental health support for all LGBTQ+ patients. It is our hope to create a standard of care that will spark a national movement to improve access and services for all people without prejudice.This role will also be responsible for providing acute case management services, referrals and linkage. The Client Care Coordinator/Case Manager will interact with the community at large and with specific allied agencies. ESSENTIAL DUTIES & RESPONSIBILITIES Reporting to the Alexis Project's Director of Mental Health and Community Engagement, the Case Manager/Client Care Coordinator is responsible for the following:Develop relationships with LGBTQ+ Community and identify resources/services for LGBTQ+ adults, youth and their familiesProvide client-centered, trauma-informed, and gender-affirming careWork collaboratively with members of inter-disciplinary teams and provide ongoing clinical consultation to DCFS and DMHDevelop relationships and ongoing communication with DCFS workers and other staffCollaborate with DCFS Clinical Social Workers and DHS staff to ensure the child, youth, or adult is linked to the appropriate level of care servicesIdentify the strengths and needs of the adult, youth and familyIdentify natural supports that exist within the family structure and the community and provide linkage to additional resources.Follow up with clients and families regarding linkage, resources, and services to ensure that their needs are being metComplete all required documentation in accordance with regulations (e.g., DMH); and enter progress notes into the electronic record (Carelogic) within 24 business hoursAssist in obtaining past health recordsFollow agency policies and proceduresParticipate in VIP in-service training programsManage patient referral list including data maintenancePerform other duties as assignedRequirementsQualifications Demonstrated experience working with LGBTQ+ adults, youth and families; demonstrated awareness of, sensitivity to, and competence in dealing with cultural and socioeconomic diversity of client populationFamiliarity with the unique healthcare needs of transgender and gender non-conforming peopleBachelor's degree and two (2) years documented experience in a mental health setting; OR Associates of Arts degree plus four years' experience in a relevant mental health setting; OR 6 years' experience in a relevant mental health settingKnowledge in working with the DMH Integrated System (IS), IBHIS and Department of Healthcare Services Medi-CalAbility to exercise good judgment and act calmly in emergency situationsBilingual in English and Spanish preferredExperience and ability to work in a fast-paced environment with good time management skillsSelf-starter, independent worker with the ability to adapt to change in a fast-working environmentGood written and verbal communication skillsValid California driver's license, driving record and personal vehicle insurance acceptable to VIP's insurance carrierExperience providing direct case management service preferredAll applications for this position will be consideredEligibility Requirements Must be willing to submit to a background investigationMust have unrestricted work authorization to work in the United StatesMust be vaccinated against COVID-19.FLSA Status Full-time; Non-ExemptBenefits & Perks Medical, Dental, Vision and Flexible Spending Accounts401(k) plan & Basic Life Insurance12 paid holidays per yearGenerous paid time off accrualTravel reimbursements at IRS rateCell phone provided for business useTo learn more about us, please visit our website: www.ViolenceInterventionProgram.org VIP Community Mental Health Center, Inc. provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, religious creed (including religious dress and grooming practices), national origin, ancestry, citizenship, physical or mental disability, medical condition (including cancer and genetic characteristics), genetic information, marital status, sex (including pregnancy, childbirth, breastfeeding, or related medical conditions), gender, gender identity, gender expression, age (40 years and over), sexual orientation, veteran and/or military status, protected medical leaves (requesting or approved for leave under the Family and Medical Leave Act or the California Family Rights Act), domestic violence victim status, political affiliation, and any other status protected by state or federal law.
