Job Summary The Care Management (CM) Assessment Coordinator II is part of the care management team and is responsible for completing health risk assessments for our members and to ensure optimal health and well-being through active engagement and helping them navigate and access health services. The CM Assessment Coordinator II supports providers and the care management team through an integrated approach to care management. Through 1:1 in-home assessments as well as telephonic outreach and assessment, the CM Assessment Coordinator II helps identify members' barriers to care and their preferences, provides education and mentoring, connects them to resources, and advocates for the individual.-- Duties Conducts in person and telephonic Health Risk Assessments for the CMC and SPD populations. Upon request from Care Managers and/or Community Health Workers, conducts other CM assessments and gather information to help the CM team inform the member's care plan. Conducts member interactions and assessments in a culturally and linguistically competent manner, leveraging the services of interpreters and assistive devices as needed. Gathers and documents the assessment responses into the clinical documentation system. (60%) Participates in care coordination and case management. Makes referrals and providing follow-up. Coordinates transportation to services and helping address barriers to services. Informs people and systems about community assets and challenges.(10%) --Advocates for the needs and perspectives of communities. Connects to resources and advocating for basic needs (e.g. food and housing). (10%) Attends regular staff meetings, on-site monthly trainings and other meetings as requested. (5%)
-- Schedules and prioritizes appointments with members/caregivers to complete assessments to ensure compliance with regulatory and departmental timelines. (5%) Performs other duties as assigned.(10%) Education Required Education Preferred Experience
Required:
At least 0-1 years of member or customer service experience within the healthcare field.
Experience in handling protected health information (PHI) and HIPAA compliance. Preferred:
Experience in face-to-face assessment or engagement activities with members. Skills
Required:
Knowledge of community resources in Los Angeles County. Comfortable working with diverse populations. Exceptional ability to connect and engage with people. Able to work in various environments including 1:1 in member's homes, clinical settings, and/ or shelters. Able to work flexible job hours. Motivational interviewing is desirable. Excellent verbal and written communication skills. Detail oriented, organized and possess time management skills. Basic computer skills. Preferred:
"Bilingual in one of LA Care Health Plan's threshold languages is highly desirable. English, Spanish, Chinese, Armenian, Arabic, Farsi, Khmer, Korean, Russian, Tagalog, Vietnamese ". Licenses/Certifications Required Licenses/Certifications Preferred Required Training Additional Information
Required:
Travel to offsite locations for work. This position is a limited duration positon. The term of this position is a minimum one year and maximum of two years from the start date unless terminated earlier by either party. Limited duration positions are full-time positions and are eligible to receive full benefits.
