Category:Patient Relations/AdvocacyJOB SUMMARY:* Responsible for
the overall responsibility for implementation of the Welcome Baby
program. Oversees, monitors, and evaluates the program to ensure
that protocols, policies, procedures, documentation, and outcomes
meet program standards. Works within a multidisciplinary team that
includes the hospital, other Welcome Baby sites in the community,
local Best Start Partnership(s), First 5 LA, and the Family
Strengthening Oversight Entity. Serves as a strong representative
of the agency and program and collaborates with community members
to strengthen the referral pathway and ensure a continuum of care
for clients and their families.EXPERIENCE/QUALIFICATIONS REQUIRED:*
Experience working in community settings, and knowledge of maternal
and child health.* At least seven years of program implementation
and supervision experience.EXPERIENCE/QUALIFICATIONS PREFERRED:* At
least two years of experience in home visitation with preference
for experience making home visits to low income families.* At least
five years of experience working in maternal child health.*
Experience and training providing reflective supervision and
assessing staff skills to identify areas that need special
attention.* Proven leadership skills and management experience,
including the ability to develop team performance and genuine
desire to coach and mentor.* Track record of managing contracts,
grants, and programmatic activities.* Knowledge of normal fetal,
infant, and toddler development; maternal and child health; and
early childhood education best practices.* Demonstrated interest
and/or strong understanding of current and emerging public health
issues.* General knowledge and familiarity with political contexts
at the federal, state, and local levels.* Knowledge of local
community resources and ability to refer families to appropriate
service providers.* Familiarity with the effects of domestic
violence, depression, perinatal depression, and parental alcohol
and/other substance abuse on family health, parenting, and general
functioning.* Experience working with low-income families and
families from diverse cultural backgrounds.* Demonstrate empathy,
warmth, and the ability to establish trusting supportive
relationships with a socio-economically and ethnically diverse
population.* Self-motivated, able to balance multiple and competing
priorities or deadlines, excellent organizational skills, and
ability to track multiple tasks as delegated to program staff.*
Strong focus on being strategic, problem solving, and solution
focused.* Ability to conduct strength based family assessment
across a broad spectrum of areas using a validated, standardized
tool administered in the prescribed manner.* Outstanding written
and verbal communication skills.* Demonstrated skills in writing
notes, correspondence, memos, and reports.* Ability to organize and
maintain complete, accurate records and file reports in a timely
manner.* Ability to work collaboratively as a member of a
multidisciplinary team.* Bilingual English/Spanish Preferred.* Must
have a reliable automobile for use on the job (mileage to be
reimbursed).* Satisfactory Driving Record, Automobile, and Valid
Automobile Liability Insurance required.* General computer skills.*
Willingness to engage in continuous learning and training.* Highly
flexible and responsive to changing environment.* Understanding or
experience with program evaluation.EDUCATION:* Masters in Nursing,
Social Work, Public Health, Public Administration, Early Child
Development or closely related field with concentration and
relevant management experience in the area of maternal and child
health and wellbeing; working knowledge of clinical social services
and community settings.LICENSURES/CERTIFICATION:* Must successfully
complete and maintain LA City Fire Card certification at the time
of hire or within the first 30 days of employment.* Must
successfully complete and maintain Certification Lactation
Education certificate within six months of hire.DUTIES AND
RESPONSIBILITIES (These are the essential job functions for this

the health or safety of themselves or others):* Carries out the
mission, vision and values established by the agency.* Oversees and
monitors the Welcome Baby Program to ensure standardized
implementation and fidelity, including adherence to protocols,
policies, procedures, and documentation.* Assists in maintaining
and improving the quality of the program by ensuring that the
following is conducted by the management team:* Bi-monthly group
reflective supervision;* Weekly individual reflective supervision
of home visitation staff;* Periodic shadowing of staff on home
visits; and* Regular chart reviews to ensure the accurate
collection of data and completion of all client documentation in a
timely and organized manner.* Monitors progress in achieving
programmatic goals and objectives within the Scope of Work.*
Prepares quarterly and year-end reports as well as agency required
reports and documentation.* Collaborates with the Oversight Entity
and First 5 LA to assist with data and evaluation needs.* Monitors
program implementation for ongoing problems and barriers and
identifies possible solutions.* Works closely with other local
providers to develop a comprehensive referral network and pathway
and track any gaps or barriers to clients being able to access
needed services.* Works with the Director of Programs and the
Accounting Department to assist with fiscal monitoring of program
and helps formulate annual budget and needed modifications
throughout the year.* Performs managerial duties including
interviewing/hiring applicants for employment; conducting
performance appraisals and professional development plans.*
Directly supervises program staff including a Clinical Supervisor,
Data Evaluation Manager, Outreach Specialist, Administrative
Assistant, and Hospital Liaisons (3), conducts administrative and
individual reflective supervision.* Works closely with the
management team to provide individual and group reflective
supervision and resolve clinical and administrative issues in a
timely fashion.* Conducts monthly staff meetings to address and
obtain feedback on program updates, needs, challenges, and
outcomes.* Works closely with the Clinical Supervisor to assess
ongoing needs for staff training; identify internal or external
professional development activities; and develop a training and
professional development plan/calendar.* Participates in twice
monthly group reflective supervision facilitated by the Clinical
Supervisor.* Participates in management team meetings and
activities; attends meetings to coordinate agency/hospital/program
activities; develops new client related activities and initiatives
as needed.* Attends First Five and community meetings to develop a
strong referral pathway and enhance the delivery of high quality
services to parents and children.* Participates in local community
meetings and develop relationships with providers and community
organizations to enhance partnerships and collaboration as
appropriate.* Shares key program data with partners and other
stakeholders on a quarterly basis or as needed, including outcomes,
demographics, and enrollment information.* Establishes a
relationship and visibility for the program with key personnel at
the hospital, including Labor and Delivery, Couplet Care, NICU, and
Social Work staff.* Oversees the accurate and timely collection of
data. Generates and analyzes monitoring and outcome reports.*
Monitors program and staff activities for compliance with the
program scope of work. Develops and monitors a quality assurance
plan as well as other internal systems to ensure compliance with
program standards and contract requirements.* Confers on a regular
basis and plans strategies with Director of Programs regarding
ongoing progress toward meeting contract goals, outreach plan,
enhanced delivery of services, and the development of a strong
referral pathway.* Maintains complete and accurate records and
files; completes required reports in a timely manner.* Collaborates
with First 5 LA and Oversight Entity in marketing efforts.* Works
closely with the Outreach Specialist, Hospital Liaisons and Parent
Coaches conducting outreach to monitor and improve prenatal
outreach, acceptance, engagement, and enrollment.* Provides time
for staff to attend periodic Peer to Peer workshops, webinars and
conference calls; and all Welcome Baby and Home Visitation regional
meetings.* Attends various agency and senior management meetings as
required* Completes all trainings as required.* Other duties as

of and follows safety practices. Understands the importance of
safety, including patient safety in the work place. Maintains a
safe environment for self and others.Actively participates in the
Patient Safety Program, including event reporting. Identifies
sentinel events/near misses and responds per defined organization
processes. Participates in education activities and process
implementation. Demonstrates advocacy for the patient/customer and
appropriately acknowledges patients, customers and visitors.The
